SNH Trauma Team Roles and Responsibilities

 

MD

Request Specialty consultation: Surgeon, Orthopedics, admitting MD (PMD, Peds, OB, ICU), aeromedical transfer

Wear and enforce PPE use (Standard Precautions) +/- lead apron for XR

Direct the resuscitation

Direct, supervise and assist with procedures

Responsible for airway (ultrasound as applicable)

Perform endotracheal intubation when GCS<8, shock

Receive/review pre-hospital report

Perform Initial & Focused Assessments

 

During Initial Assessment:

A: Listen to airway assessment

B: Listen to bilateral breath sounds

C: Assess pulse quality, look for hemorrhage

D: Evaluate for disability

E: Expose & assess back (palpate or log-roll)

 

During Focused Assessment:

Perform Head to Toe assessment, including thorough head assessment

After completion of initial & focused assessments, obtain medical history, including current medications, allergies, tetanus immunization

Obtains blood specimens per femoral arterial puncture

 

Ongoing Assessment:

Responsible for gathering patient assessment information

Develop plan of care, including test and procedures

Monitor patient response to interventions

Communicates to RN, plan of care and results of tests and procedures

Control traffic with TN #1

Control noise volume

Communicates with Pastoral Care

Activate helicopter pad landing lights as needed

 

TN #1

Primary Trauma Room Nurse / Recorder, will ultimately care for patient

Document all resuscitation data on the Trauma Resuscitation Record

Document notification and arrival times of all team members and consultations

Verify receipt from Blood Bank of four units of O-Negative blood for Trauma Alerts

Wear and enforce PPE use (Standard Precautions) +/- lead apron for XR

Control traffic with MD

Control noise volume

Apply patient’s yellow lab identification band (match triage tag if applicable)

Ensure accuracy of lab and X-ray orders

Obtain medications

Assume primary care of patient after completion of initial and focused assessments and interventions

Monitor patient during transport and procedures outside of ED

Document location of patient valuables with security or the family

 

TN #2 Left Side Support Nurse

During Initial Assessment:

obtain pulse rate

obtain respiratory rate

ensure existence of one IV, insert one if needed

During Focused Assessment:

take initial manual blood pressure

apply pulse oximetry cables

take temperature

Administer medications

 

TN #3 Right Side Support Nurse

During Initial Assessment:

monitor and maintain IV(s)

draw blood specimen as needed

During Secondary Assessment:

            apply cardiac monitor leads

            apply automatic BP cuff (unless unable to utilize right arm)

maintain patient warmth, cover with blanket or warming blanket

Assist with medication administration, if needed

Set up Level I Fluid Infuser

Administer blood products

May insert foley catheter and/or gastric tube placement after Focused Assessment

 

 

ED Tech

In absence of unit clerk page Trauma Team members as below

CPR

Cricoid pressure

Manual C-spine stabilization for intubation

Remove clothing

Assist with IV access and blood draw as needed

Label blood tubes

Enter laboratory and X-ray orders

Label all ED paperwork/forms

Transport blood and urine specimens to lab tech or tube transport system

Remove jewelry after initial and focused surveys and interventions

Bag clothing and valuables.  Make sure TN#1 documents valuables destination.

May insert foley after focused assessment.

Cleanse wounds

Tetanus booster as indicated

Assist other team members

Assist with patient transfer

 

 

Unit Clerk

Page Trauma Team members after hours: CT tech, RT, Pastoral Care

 

 

Laboratory Technician

Draw blood specimen as needed

Bring O-negative blood units, deliver to room

 

 

Respiratory Therapist

Respond to Trauma Room for Trauma Alert page

Provide on-going ventilations

 

 

Radiology Technologist

Respond to Trauma Room following Trauma Alert page

 

 

Admission Clerk

Obtain patient demographic information

Prepare admission paperwork and patient’s hospital identification band

Present registration information to TN#1

Apply patient’s hospital identification band

 

 

 

Prehospital Care Providers  

Provides pre-hospital information to MD

Provide patient’s demographic information

Provide on-going ventilation

Cricoid pressure

Manual C-spine stabilization

CPR

Assist with additional IV access

Assist with spinal stabilization for log-rolling patient

 

 

Pastoral Care

Communicate with family and escort them through ED when appropriate

Provide and coordinate spiritual support to patient and family

Assist as liaison between the medical team and the family

Recommend or initiate Critical Incident Stress Debriefing program

 

 

Security

Respond to Emergency Department when Trauma Team activated

Secure helicopter landing pad as needed

 

 

Municipal Police or Deputy Sheriff

Communicate with TN#1 after Initial Assessment

 

 

Public Relations [Nursing Supervisor]

May obtain preliminary patient information

May obtain additional patient information from Trauma Team member when appropriate

Responsible for managing media inquiries

Enter Trauma Room when requested by Trauma Team member
